slow, progressive stretching exercises to relieve spasms of hyper function; firm pressure to outer lip in lateral and superior/inferior directions
Which exercise would be used for a pt with hypotonic reduced labial closure?
range of motion exercises (pursing, retracting, relaxing lips); strengthening exercises holding candy or sticks held between lips.
Which exercise is done for reduced palatal lifting?
straw sucking tasks using a hierarch of difficult materials (thicker textures and smaller straws)
What are some pharyngeal rehabilitative exercises?
effortful swallow, masako maneuver, vocal adduction exercises, mendelsohn maneuver, Shaker exercise, NMES, EMST
What is an effortful swallow?
patient swallows HARD.
Who should use an effortful swallow?
pt with decreased laryngeal elevation and pharyngeal constriction
What does an effortful swallow do physiologically?
improves laryngeal elevation and pharyngeal constriction over time AND immediately.
What is the masako maneuver?
tongue hold maneuver
What are the effects of a masako maneuver?
immediate increase to the anterior bulging of the posterior pharyngeal wall to meet the tongue base; no long term benefits documented
Who would you use a masako maneuver with?
not sure? tongue base retraction; pharyngeal contraction
What is the mendelsohn maneuver?
pt holds swallow at highest point, holds for several seconds before letting go.
what is the rationale behind the mendelsohn maneuver?
designed to increase the extent and duration of hyolaryngeal elevation and thereby increase the duration of PES opening
What is the Shaker exercise?
head lifting maneuver
What is the isotonic shaker exercise?
pt lifts head and holds for one minute three times.
