Rehab Final Flashcards

1
Q

What are the risks of a patient being obese and what percentage of dogs and cats are obese?

A

50% of patients are obese
-increased risk of arthritis, urinary tract disease, cardiac disease
-increased risk of some cancers, diabetus mellitus in cats,
-reduced longevity

Q

How do you assess BCS and MCS?

A

Goal is 5 or 4/9.
-5 is ideal -> ribs are palpable with minimal fat covering. Noticeable waist behind ribs. Slight abdominal tuck. Abdominal fat pad absent

-Muscle condition scoring
=normal, mild loss, moderate loss or severe loss
-they have significant muscle loss if they are overweight
-graded 1-9
-evaluate muscle over the spine, shoulder blades, skull and hips

Q

How do you design a feeding regime for a patient that needs to lose weight?

A

10% bw per bcs on 9 point system
-If we want her to be a target of 5 and she is an 8 at 75 lbs, 8-5=3
10% of 75 lbs is 7.5 lbs
3x7.5= 22.5 lbs so substract that from 75

Q

How do we estimate energy needs for weight loss?

A

DER=RERX1.0
-feed 80-100% of RER for ideal / target body weight
RER = 70(BWx30) +70 kcal
90% of calories come from food and 10% from treats

Q

How do we monitor a patient for weight loss/gain?

A

Monitor every 2 weeks
for bcs and mcs
-we want them to lose 1-2% per week
-if more than 2% loss add 10-20% food volume
-if less than 1% reduce food volume by 20%
-continue to monitor after ideal bcs is achieved -

Q

How do you evaluate a nutraceutical for efficacy and safety?

A

Look at peer reviewed published studies
-dont rely solely on testimonials
-Label:
-if animal product then regulated under Fedca, guaranteed analysis, guidelines for use, 3rd party testing

-Manufacturer
-responsive to questions, quality control

Q

What are the different weight loss diets and why are they helpful for obese patients?

A

Weight loss dry and adult maintenance canine dry
-tells you how much protein, fat, fiber and calories are in per cup
-enhancing msucle mass and satiety

Q

What is a stem cell?

A

Its an unspecialized cell that is normally present in tissue that can be harvest and multiplied in a lab
-they have regenerative capabilities
-they modulate inflammatory responses
-have a range of applicaitons

Q

What is PRP?

A

Platelet rich plasma -> prepared from the patients blood glucose
-alpha granules -> growth factors (anabolic, transforming growth factor beta, platelet derived growth factors)
-cytokines and chemokines
-stmulate cellular proliferation (tissue healing)

16
Q

How do you make PRP?

A

Collect blood (anticoagulant citrate dextrose A, ratio 1-7)
Prepare blood (centrifuge)
Adminsiter (sterile technique, under some sedation

17
Q

What is autologous conditioned serum?

A

Autogenous
Induces WBC to produce anti-inflammatory cytokines

18
Q

What is the mechanism of action of ACS?

A

Cytokines ->proinflammatory vs anti-inflammatory

19
Q

How are stem cells prepared?

A

Collection -> surgical collection of adipose tissue
bone marrow aspirate

Isolation ->gradient density centrifugation

Expansion -> population doubling until sufficient numbers are reached

20
Q

How do you make ACS?

A

Collect blood -> sterile technique, collect with butterfly catheter into commercial synringe

Incubate ->37 Degrees, 7 hours for 10 ml syringe,

Centrifuge -> sterile technique, collect serum, store at -20 until needed

21
Q

What are the uses for stem cells?

A

Mesenchymal ->regenerative, anti-inflammatory, immunomodulatory, and trophic functions

Tissue healing -> regeneration of tissue

Anti-inflammatory properties -> produce anti-inflammatory cytokines, able to module inflammatory reponse,

-Modulate lymphocyte reponse

-drug testing and disease

-bone/tendon regeneration studies

22
Q

What are the uses for PRP?

A

Stimulate cellular proliferation -> tissue healing

-OA

-Bone healing

Tendon injury

23
Q

What are the uses for ACS?

A

-Anti-inflammatory
-OA in horses
