What are the peri-articular causes of ankle/foot pain?
Plantar fasciitis
Subcalcaneal bursitis
Achilles tendinitis
Tibialis post rupture
What is plantar fasciitis?
Enthesitis at insertion of plantar fascia tendon into calcaneum
How does plantar fasciitis present?
Localised pain on walking/standing
-worse when weight bearing in the morning
Midline tenderness
What causes plantar fasciitis?
Idiopathic, occurring alone
Occuring w/ seronegative spondyloarthropathies
What is the management of plantar fasciitis?
Heel pads
Reduced walking
Splinting of foot in dorsiflexed position
Most spontaneously recover w/i 1yr
What is subcalcaneal bursitis?
Pressure induced bursa under heel
What causes subcalcaneal bursitis?
Pressure induced
-often due to tight shoes
How does subcalcaneal bursitis present?
Localised pain on walking/standing
-worse when weight bearing in the morning
Midline tenderness
What are the signs on examination of subcalcaneal bursitis?
Compression of heel pad from sides is painful
What is the management of subcalcaneal bursitis?
Heel pads
Reduced walking
What is achilles tendinitis?
Enthisitis at insertion of Achilles tendon into calcaneus
What causes achilles tendinitis?
Trauma
Compression w/ seronegative spondyloarthropathies
How does achilles tendinitis present?
Pain behind heel & leg
What is the management of achilles tendinitis?
Raising shoe heel (reduces pain)
Low pressure corticosteroid injection
How does tibialis posterior rupture present?
Sudden pain
Valgus deformity
Tenderness around medial malleolus
