What is Developmental Dysplasia of the Hip?
Spectrum of disorders ranging from dysplasia to dislocation
How is DDH normally identified?
Newborn check using Barlow & Ortolani manoeuvres
How can DDH present?
Abnormal gait/limp
Asymmetry of skinfolds around hip
Limited abduction of hip
Shortening of affected leg
How common is DDH?
1.3/1000 live births
How is DDH managed?
Splinting to keep the hip flexed & abducted for several months
What are the risk factors for DDH?
Sibling hx Females Firstborn Breech delivery Oligohydramnios Neuromuscular disorders
What causes flat feet in toddlers?
Flat medial longitudinal arch Fat pad (disappears w/ age)
What are the causes of a painful, fixed flat foot in older children?
Tendo-Achilles contracture
Tarsal coalition
Inflammatory arthropathy
How are flat feet treated?
Footwear/arch support
What is forefoot adduction?
Adduction deformity of highly mobile forefoot
How is forefoot adduction managed?
Passively correctable by holding heel in normal positions
Treatment not required unless
-persists >5yrs
-symptomatic
What is Scoliosis?
Lateral curvature in frontal plane of spine
-vertebral bodies rotated
How does Scoliosis present?
Rib asymmetry
Pain-free
Cardioresp failure
What are the causes of Scoliosis?
Idiopathic
Congenital (VACTERL)
Secondary
What are the management options for Scoliosis?
Mild - resolve spontaneously
Severe - managed in specialist spinal centres w/ bracing/surgery
