Mderate -accessory muscle use , tacheal tug , unable to complete sentances ,90-94, 100-120HR ,
Severe- ALOC ,agitated, Soft/absent breath sounds ,talk in words . 120< HR and <90%
Nausea

Dehydration
mild- decreased urine out put , mild tachycardia
Moderate -low skin tugor, lethargic, sunken eyes
severe -poor perfusion, slow cap refill , tachycardia
Red flags Infants , 65 yo, diabetes ,renal failure , sepsis ,over seas travel
COPD
Inability to move not eat or sleep cannot manage homecare or meds ALOC worse hypoxia new arrythmia CORB - confusion oxygen 90% RR 30 and BP 90 0r 60
Gastro
Distenstion bile stained vomit fever >39 blood in vomit or stool abdo pain headache
Sepsis
Fever cough/sputum abdominal pain /distention dyuria reddness,swelling ALOC <35 or >39
PRECARE
- Problem – identify the ethical problem
- Reconnaissance – get the facts
respect for autonomy, nonmaleficence ,benifits and justice- Code – consider your professional code of conduct
- Alternative – consider an argument for an alternative course of action
- Regulations – consider the relevant legal regulations
- Evaluate – evaluate the various considerations and make your final decision

• Any strange feelings anywhere else?
• Any palpitations?
• Dizziness? Any fainting?
• Difficulty breathing at night or when lying down?
• Any pain or strange symptoms in your calves?
• Any risk of clotting (long-distance travel, OCP, known clotting tendency)?
• Any family history of heart disease, heart attacks, strokes, high BP?
• Do you been diagnosed with any of the above?
• Cardiac risk factors (male, age > 50yrs, obesity, smoking, hypertension, diabetes, etc)
