Differentials for viral conjunctivitis:
Periauricular adenopathy
Follicles
Punctate keratitis
Infiltrates
Difference between papillae and follicles?
Papillae have a central blood vessel (1mm < Giant papillary conjunctivitis - allergic conjunctivitis)
What is chemosis?
Oedema of the conjunctiva
Differentials for chlamydial conjunctivitis:
> 3 weeks
Fluorescent Ab stain and Giemsa stain
Normal eyelids
Follicles
Symptoms of herpes eye disease:
Severe monocular pain
Red, tearing, photophobic eye
Blurred vision
Signs of herpes eye disease:
Vesicular blepharitis (raised lesions on eyelid)
Follicular conjunctivitis
Preauricular adenopathy
Staining epithelial dendrites
Signs of zoster:
Hutchinson’s sign (side of nose)
Acute skin rash, fever and malaise
Usually resolves in a week
Complications of zoster:
Keratitis and corneal lesions
Optic neuritis and extra ocular muscle palsies
What is episcleritis:
Normal VA/mild blur with grittiness and local redness
No corneal involvement or discharge
Treat with topical NSAIDs/steroids
What causes scleritis?
Herpes zoster ophthalmicus RA Relpasing polychronditis Wegener granulomatosis Polyarteritis nodosa SLE
Symptoms of scleritis:
Gradual onset severe pain (dull and deep)
Photophobia
Tearing
Normal VA/slightly blurred
Signs of scleritis:
Scleral hyperaemia (tender) Necrotising scleritis (pale areas within red zone)
Symptoms of chalazion:
Chronic, painless, mildly tender, round swelling of the eyelid
Signs of cellulitis:
Fever Lid oedema Chemosis Proptosis Decreased eye movements +/- diplopia
RFs for dry eyes:
Beta blockers RA and SLE Sjorgen's syndrome Thyroid eye disease Facial nerve palsy
