Leads: II, III, aVF
Inferior Wall (Right Coronary Artery - RCA)
Common Conditions: Inferior MI, RCA occlusion
Leads: V1-V4
Anterior Wall (Left Anterior Descending - LAD)
Common Conditions: Anterior MI, septal infarct (V1-V2), large anterior infarct (V3-V4)
Leads: I, aVL, V5, V6
Lateral Wall (Left Circumflex Artery - LCx)
Common Conditions: Lateral MI
Leads: V1, V2
Septal (Left Anterior Descending - LAD)
Common Conditions: Septal infarct
Leads: Look for ST depression in V1-V4 (mirror image of ST elevation in posterior leads)
Posterior Wall (Posterior Descending Artery - PDA, branch of RCA or LCx)
Common Conditions: Posterior MI (confirm with posterior leads V7-V9)
Leads: V4R (right-sided lead)
Right Ventricle (Right Coronary Artery - RCA)
Common Conditions: Right ventricular infarct (common with inferior MI)
ST Elevation
Acute infarction (STEMI)
ST Depression
Ischemia or reciprocal changes from an infarct elsewhere
T Wave Inversion
Ischemia, evolving MI, pericarditis
Pathologic Q Waves
Old infarction
Wide QRS
Bundle branch block, ventricular rhythms
Prolonged PR Interval
First-degree AV block
Short PR Interval
Pre-excitation syndromes (e.g., WPW)
