What are automatic RX trip entry conditions?
What are the manual required RX trip entry conditions?
What are immediate actions and contingencies for RX trip PB?
1) If RX fails to trip depress backup CRD power supply breaker PBs on C03 (A-501 and B-631)
2) If backup PBs fail, Manually insert rods and send operator to open CRD breakers
3) If more than one rod fails to FULLY insert or power not dropping perform RT-12
What are the immediate action contingencies for the turbine?
1) If 125V DC Bus D01 de-energized by turbine trip solenoid light out and breaker position indication for A3 and odd bus 480v on C10 and either D01 undervoltage alarm or D01 < 105v locally then transfer D11 to emerg supply D02 on C10 and perform loss of D01 AOP in conjunction.
2) If either SG < 900#, actuate MSLI for affected SG, actuate EFW, check SCM adequate and perform RT-6 (EFW and MSLI). Go to Overcooling or LOSCM.
What immediate action contingencies are required for loss of SCM?
1) If < 2 minutes then trip all RCPs. If > 2 min then leave RCPs running.
2) Perform LOSCM or Degraded Power
What steps are performed for the primary plant after immediate actions?
CV-1223 (orifice bypass) demanded to zero, BWST outlet open, PZR level setpoint to 100” if emergency boration not in progress.
What steps are performed if bus A3 or A4 is de-energized?
1) Check for LO relay trip alarm
2) Check EDG running (RT-21) -> If not running then investigate and place AAC generator on bus using 1107.002.
3) Reset DG output breaker anti pump feature by taking to PTL if diesel running and output breaker open. If EDG output breaker won’t close then check A1/A2 energized and attemp to close A-309/A-409 using sync switch.
What actions are performed if A1 or A2 de-energized and needed?
1) Check LO relay alarm clear.
2) Check SU1 voltage >/= 22KV
3) Energize dead bus from SU1 using sync switch. If fails to close reset anti-pump.
3) If unable to feed from SU1, energize A1/A2 from SU2 using Att E of 1107.001 for loading and energize loads as necessary.
What is required for SU2 transformer to be considered available?
1) Voltage Reg out of service with voltage >/= to 162.5 KV
OR
2) Voltage Reg in service in automatic with 3% voltage reduction disabled and voltage >/= 155.4 KV.
3) Unit 2 loads on SU2 are BOTH vital loads only or = 4.4 MVA
4) One or Both 161KV lines in service (Russ or P Hill)
What actions are taken if normal makeup and seal injection not in service?
1) Isolate L/D (CV-1221 or 1214/1216)
2) If Seal Inj and Seal cooling both lost and not restored in 2 min then Trip RCPs, monitor for reverse rotation and perform RCP AOP
3) Restore using RT-1
What actions are taken if SG levels > 410”?
1) Trip Both MFPs
2) Place overfilled SG EFW steam supply in man and close (CV-2667/2617)
3) Perform RT-5 for EFW
4) If SG pressure < 650#, trip all condensate pumps
What IA pressure is checked and what actions are taken if it’s less than 35 psig?
IA header pressure is checked > 75#
If < 35#
1) Actuate EFW and MSLI for both SGs and perform RT-6.
2) Perform loss of IA in conjunction
What actions are performed if NNI/ICS power not available per RT-20?
1) Perform AOP in conjunction if loss of NNI power
If ICS AC or DC then,
1) Actuate EFW and perform RT-5
2) Trip both MFPs
3) Perform ICS AB Ops in conjunction.
What transition occurs if SG pressure < 900 psig?
Transition to Overcooling
What actions are taken for MSSV open?
1) Attempt to reset by adjusting opening SG TBV in hand until alarm clears or 700 psig is reached.
2) Return to auto
3) If fails to restore above 900# go to Overcooling
4) GAG MSSV
