What class of drug is ramipril?
ACE inhibitor
What is the mechanism of action of ramipril?
A competitive inhibitor of the angiotensin converting enzyme - inhibits the vasoconstrictor angiotensin II - leads to smooth muscle relaxation and vasodilation
What are the indications for ramipril?
Hypertension
Symptomatic heart failure
Secondary prevention of HF post-MI
Type 1 diabetic nephropathy
What are the typical starting and maintenance doses of rampiril?
Starting - 2.5mg
Maintenance - 10mg
How is ramipril titrated?
Starting dose of 2.5mg - dose is increased at interval of 2-4 weeks, up to a maximum of 10mg
What are the common side effects of ramipril?
Dry cough
Hyperkalaemia
First dose hypotension
What are the important side effects of ramipril?
Acute renal failure
Cholestatic jaundice
Angioedema
Hypersensitivity reactions
What are the cautions for ramipril?
Renal impairment
Patients taking diuretics
Severe aortic stenosis
Avoid in pregnancy
What are the contraindications to ramipril?
Concurrent use with aliskerin in patients with eGFR less than 60, or diabetes
History of angioedema associated with ACE inhibitor use
Hereditary or idiopathic angioedema
