What is a chief complaint?
A concern or reason why a patient seeks chiropractic care
Describe present problems/illness
Detailed data collected from the clinical interview regarding a patients chief complaint (LMNOPQRST)

Illnesses, Medications, Injuries, Surgeries, and Transfusions (IMIST)
Activities of daily living that may impact their health
Exercise, diet, alcohol, tobacco, work, hydration
What is a review of systems?
Ask the patient to check symptoms they’ve had recently that are associated with various systems of the body
First 6 questions to always ask the patient
1) What do you think caused the pain?
2) When did it first appear?
3) Did it appear gradually or sudden?
4) Have you experienced anything like this before?
5) Can you point to where it hurts you?
6) Does the pain radiate anywhere else?
Middle 6 questions to ask the patient
7) Can you describe the feeling (dull, sharp, throbbing, burning, constant, etc.)?
8) Do you have any discomfort anywhere else?
9) Can you rate the pain from 1-10?
10) What percent of the day is the pain present?
11) Is the feeling getting better, worse, or staying the same?
12) What makes it better?
Last 6 questions to ask the patient
13) What makes it worse?
14) Have you noticed any other changes in bodily function?
15) Has this affected your daily living?
16) Have you taken any OTC, medications, vitamins, supplements, to help?
17) Have you tried other professional care?
18) Anything else you would like to discuss or add?
