A
  1. How old is the patient
  2. Where is the pain located
  3. Quality(colicky, vs sharp)
  4. Duration of pain
  5. Pain like this before
  6. Associated GI sx
  7. Urinary sx
  8. Systemic sx
  9. Hx of trauma
  10. Lmp, sexually active, vaginal discharge
  11. Fever
  12. Hx of chronic illness

A
  1. Vitals
  2. How old
  3. Respiratory status (tripoding, signs respiratory distress)
  4. Preceding illness
  5. Immunization status
  6. Trouble swallowing, pain with swallowing, neck stiffness, trismus, drooling, change in voice

Questions to ask for seizure

A
  1. Describe the event- AMS, extremity movements, eye deviations, lip smacking, cyanosis
  2. Hx of seizures
  3. Fever or associated sx
  4. Trauma
  5. Concern for ingestion
  6. LOC (no LOC with partial sz)
  7. How long did it last
  8. Birth Hx
  9. Pmhx (development, meds, Hx head trauma)
  10. Family Hx

A
  1. Hx of trauma
  2. Hx of re cent surgery?
  3. Medications? (ie nasal sprays, etc)
  4. Bleeding unilateral?
  5. How long did it last?
  6. What did they try to do to stop the bleeding?
  7. Hx of bleeding problems?

  2. Sudden (choking) or gradual onset?
  3. Associated sx

Questions to ask for suspected ALTE

A
  1. Describe episode-apnea, color change, change in muscle tone, choking, gagging
  2. Who witnessed the episode
  3. Any family Hx of SIDS
  4. ROS- fever, abnormal movements (sz)
  5. Pmhx- prematurity, Neuro problems, OSA
