Q

What should a nurse do if late decelerations of the fetal heart rate are observed?

A
  • FIRST: d/c oxytocin!!!
  • Turning the client to the left side will aid in blood flow to the placenta.
  • Giving the client oxygen will help provide additional oxygen to the fetus.
  • Increasing the primary IV fluid infusion will give more volume for transfer of oxygen to the fetus.

Q

Systematic desensitization and phobia disorders

A
  • phobic disorders are learned responses; learned responses can be unlearned through certain techniques, such as behavior modification;
  • systematic desensitization is a form of behavior modification; is a strategy used in conjunction with deep muscle relaxation to decrease the extreme response to anxiety-producing situations as they are gradually exposed; then exposure is increased; goal is to eradicate the phobic response by replacing it with the relaxation response

16
Q

prognosis of bacterial meningitis

A

if treated early, good prognosis; may be complications and long-term effects (seizure disorders, hydrocephalus, impaired intelligence, visual and hearing defects); therapeutic response

brain damage: “It is an unlikely possibility, but if your child doesn’t develop normally, your health care proivder will help you with any problems.”

17
Q

When fluid in the water-seal chamber does not fluctuate, what might this indicate?

A

fluctuations stop with re-expansion of lung,

x-ray will confirm

18
Q

What are the nutrition needs following surgery?

A

need to have increased protein, calories, and Vitamin C for wound healing

19
Q

What is the most common complication following a myocardial infarction?

A

dysrhythmia, with ventricular types being the most serious

20
Q

What does pulse increase with acute asthma attack indicate?

A

pulse increase is due to decrease in oxygenation of tissues

21
Q

How should RN speak to elderly client with dementia?

A

Speak slowly in a face-to-face position

schedules need to be routine, reinforced, and repeated; flexibility leads to confusion

reminiscence and life reviews help client resume progression through grief process associated with disappointing life events, and increases self-esteem

22
Q

When is there an Rh-incompatibility problem?

A

The Rh-negative woman who conceived with an Rh-positive man. The woman has Rh antibodies.

Rh-positive dominant, fetus will be Rh-positive, Rh antibodies from the mother will break down fetus’s blood cells

incompatibility only seen with Rh-negative woman

23
Q

Buck’s traction

A

immobility is a leading cause of problems with Buck’s traction; important to turn client to unaffected side q2h

head of the bed should be elevated 15-20° because the supine position can increase problems with immobility

24
Q

When pulling a nurse from another unit, what is important when considering patient assignment?

A

Give stable client with expected outcome!

Pt that requires frequent assessment, monitoring or teaching should not be given to a RN unfamiliar with pt population.

25
Q

initial indications of hepatic dysfunction

A

Fatigue and dark urine

26
Q

signs and symptoms of aspirin overdose?

A

nausea, nosebleed, hyperventilation, Tinnitus and gastric distress

27
Q

purpose of wet-to-dry dressing

A

to débride incision

remove dry dressing so wound debris and necrotic tissue are removed with old dressing

28
Q

how many blocks can a 2-year-old child stack?

A

6-7

29
Q

What s/s are seen with circulatory overload?

A

early stages will show a change in the character of respirations; dyspnea, cough, edema, hemoptysis

BP will initially increase and then fall due to congestive heart failure, doesn’t fluctuate; body temp decreases

30
Q

As a nurse leaves a droplet precaution room, the nurse should take which action?

A

Wash hands, remove the mask, and throw the trash in a container inside the room.

hands should be washed before removing mask to prevent transfer of microbes to face

31
Q

normal specific gravity

A

specific gravity normal (1.010-1.030)

32
Q

What is observed in a pt with an open pneumothorax?

A

sucking sounds with respirations at a chest wound site and tracheal deviation toward the uninjured side

in an open pneumothorax, air enters the pleural cavity through an open wound;

placing a sterile dressing loosely over the wound allows air to escape but not re-enter the pleural space

33
Q

What type of breathing is needed in the transition phase of first stage of labor?

A

rapid shallow breaths needed (pant breathing)
