QT5 Flashcards
What dietary changes are needed for a flaccid bladder following a spinal cord injury?
fluids that promotes acidic urine, minimizes risk of urinary tract infection and stone formation; such as cranberry, prune, tomato juice, bouillon
avoid citrus juices, excessive amounts of milk, and carbonated beverages which make alkaline urine
symptoms of viral conjunctivitis
extreme tearing, redness, foreign body sensation are symptoms
HIGHLY CONTAGIOUS
bromocriptine
Ergot alkaloid
Bromocriptine is used to treat certain menstrual problems (e.g., amenorrhea) in women and stops milk production in some men and women who have abnormal milk leakage. It is also used to treat infertility in both men and women that occurs because the body is making too much prolactin.
TAKE W/ MEALS TO PREVENT GI UPSET
What should a nurse do if late decelerations of the fetal heart rate are observed?
- FIRST: d/c oxytocin!!!
- Turning the client to the left side will aid in blood flow to the placenta.
- Giving the client oxygen will help provide additional oxygen to the fetus.
- Increasing the primary IV fluid infusion will give more volume for transfer of oxygen to the fetus.
What does a pillow under the knee do for the pt?
obstructs venous flow, increasing chance for thrombus formation
What does elevating an extremity do?
promotes venous return and decreases venous pressure, relieving pain and edema
When does ambulation following a surgery begin?
ambulation begins after the start of antocoagulant therapy, otherwise movement can cause pulmonary emboli
Systematic desensitization and phobia disorders
- phobic disorders are learned responses; learned responses can be unlearned through certain techniques, such as behavior modification;
- systematic desensitization is a form of behavior modification; is a strategy used in conjunction with deep muscle relaxation to decrease the extreme response to anxiety-producing situations as they are gradually exposed; then exposure is increased; goal is to eradicate the phobic response by replacing it with the relaxation response
Biofeedback
person learns to control the autonomic nervous system; is usually more useful for reducing stress associated with physiologically based disorders
What are clients with Cushing’s syndrome are prone to?
fluid overload and CHF due to sodium and water retention
Impetigo
How is it treated?
highly infectious superficial bacterial infection
treated with systemic antibiotics, antibacterial soap
loosen scab with Burrow’s solution compress; gently remove, topical ointment
How should pt be positioned for wound dehiscence?
low Fowler’s (15 degrees); reduces stress on suture line, may be placed supine with hips and knees bent
Why are drains placed during surgery?
to prevent accumulation of drainage in wound
A client in her third trimester of pregnancy reports epigastric pain. What does this inidicate?
usually indicative of an impending convulsion
cimetidine (Tagamet)
histamine H2-receptor antagonist that inhibits stomach acid production
common side effect is diarrhea, usually subsides
acne-like rash is possible, call HCP
prognosis of bacterial meningitis
if treated early, good prognosis; may be complications and long-term effects (seizure disorders, hydrocephalus, impaired intelligence, visual and hearing defects); therapeutic response
brain damage: “It is an unlikely possibility, but if your child doesn’t develop normally, your health care proivder will help you with any problems.”
When fluid in the water-seal chamber does not fluctuate, what might this indicate?
fluctuations stop with re-expansion of lung,
x-ray will confirm
What are the nutrition needs following surgery?
need to have increased protein, calories, and Vitamin C for wound healing
What is the most common complication following a myocardial infarction?
dysrhythmia, with ventricular types being the most serious
What does pulse increase with acute asthma attack indicate?
pulse increase is due to decrease in oxygenation of tissues
How should RN speak to elderly client with dementia?
Speak slowly in a face-to-face position
schedules need to be routine, reinforced, and repeated; flexibility leads to confusion
reminiscence and life reviews help client resume progression through grief process associated with disappointing life events, and increases self-esteem
When is there an Rh-incompatibility problem?
The Rh-negative woman who conceived with an Rh-positive man. The woman has Rh antibodies.
Rh-positive dominant, fetus will be Rh-positive, Rh antibodies from the mother will break down fetus’s blood cells
incompatibility only seen with Rh-negative woman
Buck’s traction
immobility is a leading cause of problems with Buck’s traction; important to turn client to unaffected side q2h
head of the bed should be elevated 15-20° because the supine position can increase problems with immobility
When pulling a nurse from another unit, what is important when considering patient assignment?
Give stable client with expected outcome!
Pt that requires frequent assessment, monitoring or teaching should not be given to a RN unfamiliar with pt population.
initial indications of hepatic dysfunction
Fatigue and dark urine
signs and symptoms of aspirin overdose?
nausea, nosebleed, hyperventilation, Tinnitus and gastric distress
purpose of wet-to-dry dressing
to débride incision
remove dry dressing so wound debris and necrotic tissue are removed with old dressing
how many blocks can a 2-year-old child stack?
6-7
What s/s are seen with circulatory overload?
early stages will show a change in the character of respirations; dyspnea, cough, edema, hemoptysis
BP will initially increase and then fall due to congestive heart failure, doesn’t fluctuate; body temp decreases
As a nurse leaves a droplet precaution room, the nurse should take which action?
Wash hands, remove the mask, and throw the trash in a container inside the room.
hands should be washed before removing mask to prevent transfer of microbes to face
normal specific gravity
specific gravity normal (1.010-1.030)
What is observed in a pt with an open pneumothorax?
sucking sounds with respirations at a chest wound site and tracheal deviation toward the uninjured side
in an open pneumothorax, air enters the pleural cavity through an open wound;
placing a sterile dressing loosely over the wound allows air to escape but not re-enter the pleural space
What type of breathing is needed in the transition phase of first stage of labor?
rapid shallow breaths needed (pant breathing)
