Posterior - most common as of 2015
Anterior - minimally invasive; rapid recovery and decreased pain post-op
Anterior Approach
THA
Pros: earlier ambulation, shorter stay at hospital, lower dislocation rate, abductors NOT cut
Cons: increased complications and infection rates
Complications
Anterior THA
Dislocations increased due to
* history of previous occurrence
* poor abductor MMT
* adductor spasticity
Posterior Hip Dislocations
More common (85%)
MOI - hip flexion, adduction, int. rotation
All motions in closed chain pronation
Ox Protocol
Post. Hip Dislocation
- Fixed Abduction - 10/20 degrees
- Limited Hip Flexion - ROM 0-60 degrees
- External Rotation is encouraged
Anterior Hip Dislocations
MOI - external rotation and extension
Increased odds in patients with hip dysplagia and femoral anteversion
Ox Protocol
Anterior Dislocation
- Fixed Abduction - 20 degrees
- Ext. Blocked - 20 degrees flexed
- Internal Rotation is encouraged (20 degrees)
Enhancing Transverse Control
- HKAFO
- Attach thigh portion to a UCBL with twister cable
