Q3: Stance Control KAFOs Flashcards
Definition
Stance Control KAFO (SCO)
controls knee motion during stance phase but allows free flexion during swing phase
Advantage
Stance Control KAFO (SCO)
- reduced gait deviations
- reduced energy needed
- improves ADLs
Indications
Stance Control KAFO (SCO)
- isolated uni- quad weakness
- knee tightness (<10 degrees)
- varum/valgum (<10 degrees)
- limited spasticity
- Max. weight = 275 lbs
- no transverse implications
- good cognition, motivation, balance
Ideal Candidate (must have…)
Stance Control KAFO (SCO)
- Isolated unilateral quad weakness
- sound contralateral limb
- no contractures OR spasticity
- good/normal hip and ankle musculature
Ankle activated
SCO Designs
Ottobock Freewalk
Becker:
1. UTX
2. Full Stride
3. Safety Stride
4. Stride 4
Engaging Mechanism
Ottobock Freewalk
Spring loaded pawl lock
Spring loaded cable activates when there is simultaneous knee extension and ankle DF
* Switch - allows for free, locked, and stance control modes
Specifications
Ottobock Freewalk
- DF PROM - 10 degrees or more
- Ankle MMT - DF (3-5) & PF (4-5)
- Joint deviation - can accommodate some valgum but no varum; 10 degrees of inversion and eversion
- freely adjustable ankle joint height
Specifications
Ottobock Freewalk
- DF PROM - 5 degrees or more
- Ankle MMT - DF (3-5) & PF (4-5)
- Joint deviation - can accommodate 10 degrees valgum and varum; 10 degrees of inversion and eversion
Engaging Mechanism
Becker UTX
Spring loaded pawl lock
Spring loaded cable activates when there is simultaneous knee extension and ankle DF
Engaging Mechanism
Becker - Fullstride, Stride4, and SafetyStride
Low profile cabling system
Contraindications are hyperextension and LLD (affected side shorter)
Hip Activated
SCO Designs
Fillauer Swing Phase Lock 2
Ottobock E-Mag and C-Brace
Engaging Mechanism
Fillauer Swing Phase Lock 2 (SPL II)
weighted pawl, internal pendulum;
locks/unlocks with angle of the hip joint
Specifications
Fillauer Swing Phase Lock 2 (SPL II)
3 modes: automatic, lock, and unlock
Increased knee flexion (151 degrees)
Can lock at 0 or 15 degrees
No foot plate necessary
Contraindications
Fillauer Swing Phase Lock 2 (SPL II)
- > 10 degrees knee flexion contacture
- hip flexion contracture
- hip weakness
- poor balance
- > 10 degrees knee hyperextension
Engaging Mechanism
Ottobock E-Mag
electonically controlled joint that is controlled by a gyroscope; independent of foot/anke
