Idiopathic 50% Inflammatory bowel disease RA, SLE Myeloproliferative disorders Lymphoma, myeloid leukaemia Monoclonal gammopathy Primary binary cirrhosis

Where does it typically occur?
A
Lower limbs
Site of a minor injury

Describe the features
A
Initially small red papule
Later deep, red, necrotic ulcers with violaceous border
May have systemic symptoms - fever, myalgia

How is it managed?
A
Potential for rapid progression, so oral steroids first line
Immunosuppression if difficult case
