What is pyelonephritis?
Inflammation of pelvis region of kidney
2 variants of pyelonephritis?
1- Acute: Ascending infection and hematogenous infection ( E.coli Mostly, Or Staph saprophyticus)
2- Chronic: Presence of Vesico Ureter Reflux( Bacilli Tubercle)
Ascending pathway of Pyelonephritis?
Bacteria due to Instrument(Catheter)—> With the help of peri-urethral skin it colonises Urethra—> into the bladder(Risk of Cystitits)—> Goes into ureter due to disruption of Vesico ureter reflux—> CLIMB UP TO THE RENAL PELVIS
Which bacteria mostly causes Acute type of pyelonephritis?
Ascending pathway by E.COLI or sometimes Staph Saprophytic
Descending pathway by Tubercular Bacilli
C/F of ascending pyelonephritis?
Fever++ and Dysuria(Burning or painful sensation while urination)
Diagnosis of Acute pyelonephritis?
Urine: Microscopic- 1)WBC in urine(Pyuria)
2)WBC cast(when WBC attach themselves to hyaline cast
Culture:
What does proteus do?
It causes Xanthogranulomatus pyelonephritis which has Lipid containing macrophages
C/F of tubercular Pyelonephritis?
Hematogenously it goes in to kidney—> Lodge—> Lead to caseous necrosis(PUTTY KIDNEY)
With time pelvis degeneration(Calcium deposition also called CEMENTED KIDNEY)
With time complete damage of renal damage( AUTONEPHRECTOMY)
