Vehicle Code Section 14602.1 requires that all police pursuits, including non- collision pursuits, be reported on a California Highway Patrol Pursuit Report CHP-187A (12-05). A SDPD Pursuit Data Sheet must also be completed. A combined CHP-187A/SDPD Pursuit Data Sheet form is available on the LAN system at F:\Templates\Patrol Based Forms \TrafficReports \CHP187 Pursuit Report.dot. A PDF version that can be printed and completed by hand is available at the same location titled NewCHP187a.pdf. The CHP-187A/ SDPD Pursuit Data Sheet form shall be prepared on all pursuits initiated or assumed by officers of this Department.
