What is the purpose of pulsed lavage?
removes loosely adhered debris, bacteria, biofilm etc.
promotes moist wound healing
What is a biofilm?
community of microorganisms and compromised of bacteria and is a major barrier to wound healing
also protected from topical agents
What is pulsed lavage considered?
mechanical debridement
What is the general standard for pressure with PL?
4-15 psi, too little will not cleanse effectively and too much can further damage wound and drive bacteria into wound
What are common numbers of PSI that are usually used?
8- shown to effectively remove bacteria
13-reduction of inflammation in traumatic wounds
What is the main difference with closed pulse lavage?
does not utilize suction and has shown to be highly effective in wound healing
What are indications for pulse lavage?
use for debridement of wounds due to arterial and venous insuff. pressure, small burns, surgery or trauma
help soften eschar for sharp debridement
Contraindications?
know your anatomy to avoid areas of exposed things like arteries etc., facial wounds, active bleeding, latex allergies
What does PPE stand for?
personal protection equipment like gloves, shoes, gowns, eyewear, mask
What temp is saline usually warmed too?
102-106 degrees if possible
How long is normal treatment session?
15-30 mins, start with necrotic tissue first
start 4-6 psi then make way up to 8-12
How many times can certain equipment be used on same pt?
5-7 times
What are advantages of pulse lavage?
PT can perform instead of MD, macertaion can be avoided, sharp debridement easier after PL
Great for cleaning tracts and tunnels etc
What is usual treatment frequency and duration?
usually once a day unless wound is severe
could be done as little as 3x per week
