ABG: Too much Heparin
PaCO2 decreases towards 0
PaO2 increase/ decrease towards room air
pH decreases toward 7.0
(same as air bubbles in sample)
A-a Gradient values
Normal - 25-65 torr on 100%
V/Q Mismatch - 66-300 torr
Shunting - >300 torr
P/F Ratio values
Normal: 380 torr or more
ALI: <300 torr
ARDS: <200 torr
Respiratory Acidosis with Hypoxemia Treatment
Initiate ventilation, increase RR
Respiratory Alkalosis with Hypoxemia Treatment
Oxygen for hypoxemia
Metabolic Acidosis
Administer Bicarbonate
Hypoxemia with high PaCO2
Poor ventilation - increase ventilation (RR)
Hypoxemia with normal or low PaCO2
V/Q Mismatch - Increase FiO2 up to 60%
Hypoxemia with FiO2 60%+
Shunting, refractory hypoxemia, venous admixture - start/increase PEEP or CPAP
Normal ABG values for Neonates
pH > 7.30
PaCO2 < 50 torr
PaO2 > 60 torr
