Pulmonary Flashcards
What is plateau pressure
Lung pressure during mechanical ventilation, measured at the inspiratory pause.
In an asthma exacerbation, what will the inspiration pressure look like?
what will the plateau pressure be like?
IP=elevated
PP= normal
In treating ARDS, the tidal volume should be kept low or high?
Tidal volume should be kept low to prevent Volume-trauma.
Patients should be positioned with the bad lung down, or the good lung down?
Good lung down (if bad lung is down, bad lung may get more perfusion and patient can become hypoxemic)
Increasing the breath rate on the vent will affect oxygenation or PaCo2?
Paco2 will be affected (ventilation). It will not change the oxygenation.
What is a SHUNT? How to treat?
Shunt is an extreme V/Q mismatch. Blood moving from right side of heart to left side without becoming oxygenated. (Venous blood to the arterial side.)
Treat with 100% FiO2 and increase the PEEP for increasing alveolar recruitment.
What is the most sensitive indicator of oxygenation at the cellular level?
SvO2 ( mixed venous blood leaving the right heart, taken at the Pulmonary artery. .)
What is the treatment for carbon monoxide poisoning?
100% FiO2 until symptoms resolve and carboxyhemoglobin level is <10%
Measurement of the elastic properties of the AIRWAYS
Dynamic Compliance
Measurement of the elastic properties of the lung
Static compliance.
Static compliance is Low
Dynamic compliance is low
ARDS
Static compliance is normal
Dynamic compliance is low
Status asthmaticus
PaCO2 is acid or base?
ACID
When PaCO2 increases, there is acidosis.
When PaCO2 decreases, there is alkalosis.
HCO3 is an acid or base?
Bicarbonate = the BASE
Too little bicarbonate = acidosis.
Too much bicarb = alkalosis.
What is the normal anion gap?
5-15
