Psychosis Flashcards
typical neuroleptics
chlorpromazine
flupenthixole
haloperidol
moa of typical neuroleptics
D2 antagonists
useful effects of typical neuroleptics
- good for positive symptoms
2. sedative
SE of typical neuroleptics
1) extrapyramidal symptoms (EPS)
2) no effect on negative symptoms
3) hyperprolactinaemia
4) aplastic anaemia
what other receptors do chlorpromazine effect?
- anti-H1 (sedative)
- anti-mAch (can slightly improve the parkinsons)
- anti-alpha1 (hypotension)
atypical neuroleptics
clozapine
Risperidone
olanzapine
moa of atypical neuroleptics
- weak antagonist of D2,
- antagonist of 5-HT2A/2C (excitatory)
- partial agonist of 5-HT1A, 4. antagonist mAChR and alpha-2 adrenoceptor
useful effects of atypical neuroleptics/3rd gen
- effects on positive and negative symptoms
- good for drug resistance
- less EPS
what is a main SE of clozapine?
agranulocytosis
SEs of atypical neuroleptics
1) Long QT interval
2) Weight Gain/Type 2 DM
3) Agranulocytosis
3rd gen neuroleptic
aripiprazole
MOA of aripiprazole
- D2 partial agonist,
- 5-HT2A antagonism
- partial agonist of 5-HT1A
SE of aripiprazole
1) hyperprolactinemia
2) hypercholesterolemia,
3) akathisia (impulsive movements – more severe than that of typicals and atypicals)
what are positive symptoms of psychosis?
auditory hallucinations, thought disorders,
delusions,
talking aloud
what are negative symptoms of psychosis?
lack of interest,
social withdrawal,
catatonia (lack of motivation to move)
