Antidepressant Therapy Certification Period & DQ
Max Cert: 1 year
Do NOT certify if:
1. Uses 1st generation antidepressant (Tricyclics - amitriptyline or Imipramine)
2. Has interfering side effects
Antipsychotic Therapy Certification Period
Max Cert: 1 year
- Used to treat schizophrenia, mood d/o, and bipolar d/o
- Includes typical and atypical antipsychotics
Anxiolytic/Sedative Therapy Certification Period and Requirements
Max Cert: 2 years
Certify if:
- If Hypnotic:
1. Short acting (half life < 5 hours)
2. Lowest effective dose
3. Short duration of use (< 2 wks)
- If Non-sedating anxiolytic
Anxiolytic/Sedating Therapy DQ
- Sedating anxiolytic
- Interfering symptoms or side effects
CNS Stimulant Therapy Certification Period and Requirements
Max Cert: 1 year
Certify if:
1. Non-DQ condition
2. No drug induced impairment
3. No tendency to increase dose
*Verify dx and request evaluation by treating MD
CNS Stimulant Therapy DQ
- DQ Condition (Narcolepsy or Hypersomnia)
- Treatment side effects
ECT Waiting & Certification Period
Waiting: 6 months symptom free following treatment
Max Cert: 1 year
ECT Certification Requirements and DQ
Certify if:
1. Completes waiting
2. Cleared
3. Not undergoing maintenance ECT
4. Tolerates tx w/o side effects
DQ if:
1. Undergoes maintenance ECT
2. Interfering side effects
Lithium Therapy Certification Period and Requirements
Max Cert: 1 year
Certify if:
1. Asymptomatic
2. Within therapeutic range
3. No impairment
ADHD Certification Period and Requirements
Max Cert: 1 year
Certify if:
1. Complies with tx
2. Tolerates tx
3. Mental health exam
ADHD DQ
- Active psychosis
- Negative symptoms
- Decreased judgement
- Decreased attention
- Suicidal behavior/thoughts
- Personality d/o - Interfering side effects
