Lithium toxicity sx
- tremors (MOST COMMON)
- slurred speech
- seizures
- delirium
- muscle weakness
- tinnitus
- blurred vision
- renal toxicity
Interventions/education for lithium
- monitor therapeutic levels
- monitor serum sodium levels
- instruct clients that therapeutic effects begin in 7-14 days
- Doses must be administered 1-3 times daily per provider prescription
- Provide nutritional counseling to include food sources for sodium
- administer food to decrease GI distress
Examples of typical antipsychotics
- chlorpromazine
- fluphenazine
- haloperidol
- thiothixene
Examples of atypical (less severe side/adverse effects)
- aripiprazole
- clozapine
- olanzapine
- paliperidone
- quetiapine
- ziprasidone
precautions/interactions for antipsychotics
- contraindicated for clients who have severe depression, Parkinson’s disease prolactin-dependent cancer, and severe hypotension
- use with caution in clients who have glaucoma, paralytic ileus, prostate enlargement, or seizure disorder
side/adverse effects for typical antipsychotics
- sedation
- extrapyramidal effects
- tardive dyskinesia
- agranulocytosis
- neuroleptic malignant syndrome
- seizures
Side/adverse effects for atypical antipsychotics
- agranulocytosis
- weight gain
- diabetes
- dyslipidemia
- orthostatic hypotension
- extrapyramidal effects
Interventions/education for antipsychotics
1 monitor for side effects within 5 hours to 5 days of administration
- advise the client of potential side effects
- monitor CBC
- encourage fluids
- stop medication for signs of neuroleptic malignant syndrome
Signs of extrapyramidal symptoms
Think parkinson’s (rigid muscles, tremor, increased salivation, slow movement, changes in posture or gait)
Tardive dyskinesia symptoms
repetitive involuntary facial movements (tongue twisting, chew motions and lip smacking, cheek puffing, and grimacing)
