What is psoriatic arthritis?
Chronic inflammatory joint disease, a seronegative inflammatory arthritis usually with mono/oligoarthritis that is not symmetrical.
What joints does PA affect?
DIP usually
What are risk factors for PA?
Psoriasis (personal or FH)
What are Sx of PA?
Stiffness and pain in peripheral joints
Dactylitis
Morning stiffness > 30 mins
Enthesitis
What are differentials for PA?
RA, gout, erosive OA
What are Ix ?
Bloods: CRP and ESR can be elevated, maybe RF +ve.
metabolic syndrome: urate, lipid, FBG
Imaging: x-ray or MRI
May need synovial fluid aspiration
What criteria?
CASPAR
What is Rx of PA?
NSAIDs
physio
intra-articular steroid injection
DMARDs or anti-TNF if severe or dactylitis
What are complications of Pa?
CVD
