What is gout
A
Hyperuricaemia causing deposition of urate crystals causing attacks of acute inflammatory arthritis

What are Sx of gout?
A
Acute mono-arthropathy and joint inflammation
Pain, stiffness, tender, red
Tophi: in pinna, tendons, joints Renal stones (uric acid)

What are risk factors for gout?
A
Age men obesity alcohol meat and protein diabetes FH

What are primary and secondary causes of gout?
A
1: genetic e.g. Lesch-Nyhan
2. cancer, ciclosporin, aspirin, CKD, thiazides

What are differentials for gout?
A
SA, RA, CPPD, haemarthroses

Ix for gout?
A
Joint aspirate: High WCC and -ve needle shaped crystals on PLM
SUA may be normal
U+Es

Q
Rx for gout
A
Acute: high dose NSAIDs and colchicine
Chronic: allopurinol, febuxostat, rasburicase, probenecid (urate lowering therapy)
