What is the pathogenesis of psoriasis?
hyperproliferation of epidermal cells with increased cell turnover, dilation + proliferation of dermal blood vessels and accumulation of inflammatory cells (esp T cells)
What factors precipitate psoriasis?
emotional stress infection alcohol smoking drugs: beta-blockers, lithium and antimalarials, steroids
What infection triggers guttate psoriasis?
streptococcal sore throat
What is the Koebner phenomenon?
trauma precipitates psoriasis
What is seen on histology of psoriasis?
thickened parakeratotic stratum corneum
munro microabsesces
no granular layer
What are the clinical variants of psoriasis?
chronic plaque
guttate
flexural
palmoplantar pustular
What is Auspitz sign?
removal of plaque reveals pin point bleeding
Manage flare of plaque psoriasis
Topical steroid + calcipotriol (vitamin D analogue)
