PRS03 - Lecture 7 - Trial Insertion Flashcards
What is the aim of trial insertion?
Try the dentures in the patients mouth - ensuring that the rest seats are engaged correctly
What 2 things are checked during trial insertion? (3:6)
Jaw relation (3)
Check contacts in maximum ICP
Check that there is natural tooth contacts
There maybe vertica or horizontal error
Appearance (6)
Shade
Mould of tooth
Centre line (mid-line)
Labial support
Smile line - edge of flange or visible clasps
Position of incisal edges
Describe the clinical steps of checking Jaw relation during Trial Insertion (7)
1) Insert wax set up of denture teeth
2) Check contacts during maximum ICP
3) Determine if there is horizontal or vertical error
4) If there is - remove teeth that are interfering with occlusion
5) Build up the spaces you’ve made with wax rim
6) Ensure the occlusal errors are not due to natural teeth on the cast framework (adjust if necessary)
7) Record jaw relation again using ZOE impression paste
Describe the clinical steps of checking Appearance during Trial Insertion (2)
If replacing anteriors - check midline, labial support and frenal relief
Show patient in mirror and make sure they are happy with aesthetics
What should you not do during Trial Insertion? (3)
Persuade patient to accept an appearance they are not happy with
Accept an incorrect jaw relation record
Finish a partial denture with a bad cast framework (leads to discomfort)
If patient complains that the upper anteriors look like false teeth - what could you do to make them appear more natural? (3)
Adjust shape of the teeth
Rotate teeth
Add stains
What could be the reason if a cobalt-chrome denture doesnt fit properly at trial insertion? (2)
Its not the metal-framework because you tried it during metal-framework try in
Its the wax or the teeth thats in the way
What aesthetic problems could a patient complain about? (6)
Shape of teeth (mould)
Colour of teeth (shade)
Position (anteriors proclined)
Colour of flanges (pigment)
DIastema (gap) not present when it was original there
Gaps present between denture teeth and gum (due to resorption)
How can you make a denture look more like natural teeth? (3)
Alter position of teeth - rotate, diastomas
Natural tooth characteristics - chips, stains, fracture lines
Flage - use different shades of pink or pigmented acryic
What labratory perscription will you make after Trial Insertion? (1)
Please process denture to finish in heat cured acrylic
