Hematuria Lower back and abd pain Fever Perineal pain Tender prostate

Acute bacterial prostatitis - treatment <35 yrs old
A
Hydrate, analgesics, bed rest
Rocephin + doxycycline
usually gonorrhea or chlamydia

Chronic bacterial prostatitis - treatment
A
Same nonpharmacological tx
DOC - Bactrim (diffuses to prostrate well)
If tx failure or recurrent - long term suppressive therapy. Consider Macrobid, if ? pseudomonas FQs (cipro or high dose levi)

Acute bacterial prostatitis - treatment >35 yrs old
A
Bactrim or fluiroquinolones if resistant
usually e.coli, pseudomonas or klebsiella
