Doesn’t exist in isolation
Examples: success, physical gains, adherence
Formative Evaluation
Monitor learning
Ongoing feedback
Examples: Ungraded knowledge checks, Group discussion, Live session input
Summative Evaluation
Evaluate learning at the end of a program or course
Compared against standard
Examples: exams, practicals, projects
Modes of gathering information
Verbal (interview, focus group, informal input as field notes)
Written (feedback form - pay attention to pt’s ability)
Survey gathering: open and closed ended (used for outcomes or learning, beliefs, values)
Performance (measurable outcomes - can be used to discover flaws)
Operational measures (attendance, no show rate, enrollement)

Identifies predisposing, enabling, and reinforcing factors relevant to the prioritized behaviors

Identifies policies, resources, and circumstances in the program’s context that may help or hinder implementation
