Principles Of Cavity Design - Remiving Caries Flashcards
What features do you want to preserve if possible?
Marginal ridge in anterior teeth
Transverse ridge in molars
Why try preserve the marginal ridge?
Weak point
What drill do you use to remove caries?
High speed drill with a friction grip diamond bur
If caries are really apparent, what is your next steps?
Use slow speed handpiece with large rose head bur to remove caries first then tidy up enamel with high speed
What occurs under an enamel lesion?
Often ballooning
Larger than it looks on the surface
What hand piece do you use for soft infected dentin?
Large rosehead bur in the slow speed handpiece
Finish preparation with spoon excavator
When to do more (how to know hem to stop)?
Soft dentinal caries still present
Enamel periphery is weak
What is infected dentine?
Infected = irreversible demineralisation and denatured layer with bacterial invasion. Very soft, moist and easy to remove with spoon excavator
What is affected dentine?
Affected = partially demineralised, leathery feel. Collagen is not denatured and contains minimal to no bacteria
Do you want to remove infected of affected dentine?
Infected
What is retention of a cavity?
The ability of a cavity to hold onto the restorative material
What are 2 methods of retention?
Physically or chemically
How can you physically retain?
The shake of the restoration, so that it resists displacement of the restoration
The restorative material is held within the tooth
How can you achieve physical retention?
An undercut within the cavity itself
Put in features such a dove tail or a groove (stops it from falling out)
When is physical retention required?
Amalgam filling for example
