pretransfusion testing Flashcards
who regulates blood product production and handling
FDA
who regulates pretransfusion testing
CMS- centers for medicare and medicaid
most common cause of transfusion mortality
TRALIs
another major concern for transfusion reactions
ABO incompatibility
clerical errors
phlebotomy/tech mismatched patient and specimen, issued wrong product, and/or administered to wrong patient
need 2 unique identifiers
name
date of birth
patient ID #/ MRN
if patient is unconscious can match through
band
specimen labeled
name
MRN
signature of drawer and witness if down by RN
Hollister number
reject a specimen if
gross hemolysis or IV contamination (don’t draw above line)
specimens good for
3 days
how long are specimen kept for after the 3 days
7 days
RBC donor unit recieved must have their
ABO/Rh type confirmed
universal type to give
O
universal plasma to give
AB
Plasma and cryo should be ABO typed but
crossmatching is not required
immunocompromised patients and infants and HLA units need units that are
irradiated
patients with 2 types on file and a negative screen with no history of antibodies can receive
either an IS crossmatch or electronic crossmatch
patients with a positive screen and/ or history of an antibody receive
AHG crossmatch
when you issue a unit you must state
unit is negative for each specific antigen, compatible, and special testing done
3 reasons AHG crossmatch incompatible
-IgG alloantibody present
-warm autoantibody present in plasma
-donor unit has a positive DAT
what happens when you can’t go through pretransfusion?
-physican approval and signature
-O neg cells and AB plasma unit patient is typed from PRESENT sample
-once know type give that specific kind
-Rh pos to men and women over 50
-any product given should be crossmatched after
massive transfusion considered
replaced entire blood volume
- within 3 hours
- more than 10 units in 24 hrs
-5 units in under a hour
tech will have to be _____ during a massive transfusion
point person and keep tract of paper work and thaw plasma
what tests are ordered after MTP is initiated
PT/aPTT
D-Dimer
Fibrinogen
CBC
CMP
T and Screen
what is considered a cycle?
6 units type O-neg, 6 units type AB plasma, 1 platelet unit
after a cycle
the point person will see if they want to continue
neonatal transfusion
infant is front typed and antibody screened using maternal plasma
AABB recommends for babies
type matched units OR type A or B cells AHG compatible
at dyer for babies they receive
Pedi-Pak
for baby crossmatching it is done using
maternal plasma
