• speech: slurred, inappropriate words, mute

if answers to all yes or unknown: sensitivity=91% & specificity=97%

• age >45
• no hx of sz disorder
• new onset neurologic symptoms in last 24 hrs
• patient ambulatory at baseline prior to event
• BSL 60-400 mg/dL
• obvious asymmetry of any of following: facial smile/ grimace, grip, arm strength
