omeprazole
what is it used for?
[1] first line treatment of Peptic ulcer for prevention and treatment, often given with long term NSAID use
[2] second line treatment for dyspepsia, GORD esp more severe cases
[3] h.pylori eradication therapy- treatment of H.pylori alongside antibiotics (2x)
MOA:
supresses gastric acid production almost completely by inhibiting the K+ H+ ATPase in parietal cells
side effects
GI disturbances
headache
increase c.difficile risk
hypomagnesia
what can hypomagnesia cause
tetany and ventricular arrhythmia
what is the problem with this medication
can disguise symptoms of gastric cancer
risk with prolonged treatment
increases risk of fractures
and monitor magnesium
drug interactions
can reduce anti platelet effect of clopidogrel by decreasing P450 enzymes
alternative for clopidogrel patients
lansoprazole and pantoprazole
best taken when
in morning, on empty stomach or with food
formulations
oral or injectable
how long is course In h.pylori
and GORD
7 days
4 weeks max before review - if not successful consider surgery
other drug interactions
decreases clearance of methotrexate so avoid.
increase exposure to citalopram
decrease exposure to atazanvir
