Postural Drainage
Method of removing secretions from the lungs
Indications for Postural Drainage
Patient on a continuous ventilator, any patient on prolonged bedrest, increased sputum production, ineffective cough, increased WOB
Contraindications for Postural Drainage
Collapsed lung, hemoptysis, MI, hypertension, post-op, aneurysm, pulmonary embolism, recent laminectomy, large pleural effusion
Precautions for Trendelenberg position
hypertension, obesity, hiatal hernia, nausea, recent food consumption, osteoporosis, DOE made worse in this position
When to stop treatment
SOB, vitals change, breath sounds are within normal limits, chest xray is clear, patient can breathe or cough independently
Huffing
when percussion is completed, take deep breath and cough up mucous by saying “HUFF” 2-3x
Breathing Techniques
Purse Lip (smell flowers, blow out candles)
Diaphragmatic (belly out, then in)
Lateral Chest Expansion
Rating of Perceived Exertion to modified borg scale
6-20 (very light to very hard) 1-10 (very light to Maximal)
Crackles
Popping (high pitched) similar to fireplace
Adventitious
Loud (low pitched) explosive crackling
wheezing
high pitched whistle
rhonchi
similar to the sound of snoring
Pleural Rub
Low pitched creaking sound or walking on snow heard during inspiration
Stridor
High pitched wheezing sound, loud and continuous
