What class is Oxytocin in?
uterotonic, hormone
What class is misoprostol in?
prostaglandin analog
What class is methylergonovine maleate in?
ergot alkaloid
What class is prostaglandin- carboprost in?
prostaglandin
What is Oxytocin used for in PPH?
used to stimulate uterine contractions to prevent and control PPH
What is misoprostol used for in PPH?
causes uterine contractions to reduce bleeding
What is methlyergonovine maleate used for in PPH?
increases uterine tone to control bleeding
What is prostaglandin- carboprost used for in PPH?
stimulates uterine contractions and reduces blood loss in PPH
What are some nursing implications concerning the administration of oxytocin?
- monitor uterine tone and bleeding closely
- assess for water intoxication (nausea, vomiting, headache, confusion, etc)
- admin slowly to avoid hypotension
What are some nursing implications concerning the administration of misoprostol?
- monitor for side effects likes diarrhea, fever, and chills
- ensure correct rectal administration
- use caution in patients with cardiovascular issues
What some nursing implications concerning the administration of methlyergonovine maleate?
- contraindicated in patients with hypertension and preeclampsia
- monitor blood pressure closely
- assess for signs of vasospam (chest pain, hypertension)
What nursing implications concerning the administration of prostaglandin-carboprost?
- contraindicated in patients with asthma
- monitor for diarrhea, vomiting, and fever
- administer antiemetics and antidiarrheals as needed
- monitor for respiratory distress
What should you educate your patient on when you administer oxytocin?
- inform the patient that this medication helps contract the uterus to stop bleeding
- explain that cramping is normal and that the medication is used routinely for PPH
What should you educate your patient on when you administer misoprostol?
- educate the patient about the possible side effects like chills and diarrhea
- reassure that these side effects usually subside after the medication has taken effect
What should you educate your patient on when you administer methylergonovine maleate?
- advise the patient to report any headache or chest pain immediately
- explain that this medication can raise blood pressure and will not be given if they have high BP
What should you educate your patient on when you administer prostaglandin- carboprost?
- explain to the patient that this medication helps contract the uterus but can cause nausea, vomiting, and diarrhea
- advise to inform the nurse if breathing becomes difficult
What dose of oxytocin would you administer to your patient?
10- 40 units in 1,000mL IV; or 10 units IM
What dose of misoprostol would you administer to your patient?
800- 1,000 mcg rectally
What dosage of methylergonovine maleate would you administer to your patient?
0.2mg IM every 2-4 hours (up to 5 doses); or 0.2mg PO 3-4 times daily
What dosage of prostaglandin- carboprost would you administer to your patient?
250mcg IM every 15- 90 min (up to 8 doses)
