Breasts
soft to filling; no redness, lumps, or cracked nipples. Mild tenderness is common as milk comes in.
Uterus
firm, midline; fundus at or 1-2 cm below the umbilicus within 24 hrs postpartum.
Bladder
non-distended; patient voids within 6-8 hrs after delivery without difficulty
Bowels
active bowel sounds; passing gas. The patient may not have had a bowel movement yet but should not be in distress
Lochia
rubra (bright red), moderate in amount, no large clots or foul odor
EPISIOTOMY/ PERINEUM
mild swelling or tenderness if a tear/ episiotomy occurred, no signs of infection (redness, discharge)
EXTREMITIES
no swelling, pain, or signs of deep vein thrombosis (DVT)
EMOTIONAL STATUS
mood stable; bonding well with baby, no signs of postpartum depression or anxiety
