Superior nuchal line, external occiptial protuberance, nuchal ligament, SPs of C7 - T12
Lat 1/3 of clavical, acromion and spine of scapula
Accessory nerve (CN XI) (motor) C3/4 spinal nerve (pain and proprioception)
Superior: elevates scapula
Middle: retracts scapula
Inferior: depresses scapula
Superior and inferior together: rotate glenoid fossa superiorly.

SPs of inferior 6 thoracic vertebra, thoracolumbar fascia, illiac crest, inferior 3-4 ribs
Floor of the intertubercular groove
Thoracodorsal nerve
Extends, adducts and medially rotates the humerus
Levator scapulae
Deep muscle

TPs C1-4 vertebrae
Medial border of scapula superior to the spine of the scapula
Dorsal scapular nerve and cervical nerve C3,4
Elevates scapula and inferiorly rotates glenoid fossa
Rhomboids
Deep to trapizues
Split into major and minor

Minor: nuchal ligament, SPs of C7-11
Major: SPs of T2-T5
Minor: medial end of scapula spine
Major : medial border of scapula, below the spine
Dorsal scapular nerve
Retract scapula, inferiorly rotate glenoid fossa, fix scapula against thoracic wall
