Initial presentation of PM and DM
Proximal muscle weakness, progressive and symmetric
Blue-purple discoloration on the upper eyelids with edema
Heliotrope rash
Erythema of the knuckles with raised violaceous scaly eruption resulting to scaling of the skin
Gottron rash
Rash on back and shoulders
Shawl sign
MC inflammatory myopathy in patients >50 years old
Inclusion body myositis
Definite diagnosis of IBM
Muscle Biopsy
80% of patients positive for this antibody have ILD
anti-JO1
Most sensitive enzyme which parallels disease activity
CK
Diagnostic of DM even in the absence of inflammation
Perifascicular atrophy
Least favorable prognosis among all myopathies
IBM (resistant to immunosuppressive therapy)
Treatment of choice
Prednisone 1mkd and tapered over 10 weeks
DM>PM
