Early inflammatory stage
Subclinical systemic inflammation begins-activation of the innate immune system
Cytokines (interleukin 6) and tumour necrosis factor alpha are elevated
Acute symptomatic stage
Onset of PMR symptoms - pain & stiffness- neck, shoulders, hips
Inflammation affects synovial lining of bursea, joints and tendons
Chronic or recurrent stage
Chronic inflammation can lead to structural damage & functional decline
Resolution stage
Treatment with corticosteroids
Without treatment - prolonged inflammation can lead to vascular damage
Giant cell arteritis- PMR patients 15-20%,
Can lead to vascular complications such as vision loss
What is it?
Polymyalgia Rheumatica (PMR) is a chronic inflammatory disorder that primarily affects older adults, particularly those over 50 years old. The exact pathophysiology is not fully understood, but it is thought to involve an autoimmune response and systemic inflammation.
It is often linked to giant cell arteritis (GCA).
