Cyanide Poisoning

A

Binds and prevents cytochrome oxidase. B12 binds to make able to be excreted renally. Causes an elevated anion gap metabolic acidosis b/c disrupts the ETC and impairs oxidative respiration. Elevated Pao2 and SVo2 (have oxygen but cant utilize it). Treatment with B12.

Acute Mountain Sickness

A

Decrease in PaO2 and PaCO2 due to increased atmospheric pressure. Chemoreceptors stimulated resulting tachypnea resulting in respirator alkalosis. Gradual reduction in bicarbonate to compensate. PPx with Acetazolamide and give supplemental oxygen

Methemogloblinemia

A

Oxidized from Fe2+ to Fe3+. O2 saturation reading 85% but PaO2 normal. Treated with Methylene blue
