Management of Complex Pain Syndrome
Lumbar sympathetic block (unilateral sympathectomy to see if pain improves); if successful, can perform neurolysis. Last resort is spinal cord stimulation or intrathecal pump with Baclofen or opioids.
Allodynia
Perceives an non-noxious stimuli as painful
Anesthesia Dolorosa
Pain in an area that lacks sensation (most likely the face)
Hyperalgesia
Increased pain response to noxious stimuli or pain out of proportion to physical findings
Intrathecal Morphine Conversion
1:10:100 (Intrathecal, epidural, IV)
Pain Fibers (A Delta and C)
A Delta- thin and transmit acute sharp tingling sensation, C fibers- small and slow which transmit dull burning sensation
Codeine
Converted to morphine. Contraindicated in children following tonsillectomy
