Tension pneumothorax
When air can come in but has no way of coming out causing high pressure in blood vessels. Also limiting venous return leading to decreased cardiac output.
Can be deadly.
-causes mediastinal shift
Hemothorax
Accumulation of blood in pleural space
Flail chest
Instability of chest wall due to ribs that have possible gotten multiple fractures and have no support
Sound of chest when there is pneumothorax and hemothorax
Hyperresonance (pneumo)
Dull (hemp)
MedicAtions
- benzodiazepines (sedatives)
- opioids antagonists (pain medication)
Benzodiazepines
(Sedative)
-lorezapam (Ativan) : used to decrease anxiety
Monitor: vital signs, (may cause hypotension or resp. Distress) & paradoxical effects (euphoria,rage)
Causes amnesiac effects and drowsiness
Opioid agonists
(Pain medications)
- morphine sulfate & fentanyl (durgesic)
- used to treat moderate to sever pain.
- acts on the mu and kappa receptors that help alleviate pain
- Assess pain every 4 hours.
- Febtanyl patch acts slower
- If respirations are 12/min or less in elderly =STOP
Objective data for flail chest
- unequal chest expansion
- tachycardia
- hypotension
- dyspnea
- cyanosis
