Tall thin male, smoking, family history

A
Underlying lung disease, smoking

Symptoms of pneumothorax (4)
A
- Sudden onset SOB
- Pleuritic chest pain
- Cough
- Fatigue

Signs of pneumothorax (4)
A
- Decreased breath sounds
- Hyperresonant percussion
- Decreased vocal resonance / tactile fremitus
- Cyanosis/low sats

Signs of pneumothorax on CXR and CT
A
CXR: linear shadow, mediastinal shift away, deep sulcus sign, effusions
CT: blebs, bullae

Management of pneumothorax, including where to place the chest tube
A
- Small - no treatment
- Secondary - needle decompression
- Large/tension - chest tube
Tension - 2nd ICS MCL
Non-tension - 5th ICS MAL
- Pleurodesis, pleurectomy
