what is a plan of care (POC)
overall management strategy of the PT to accomplish the stated patient goals
Episode of care
specific diagnosis for which they are coming to the discharge for that diagnosis or set of events
Direct interventions
the main component of Tx in enough detail for another PT/PTA to follow
ex. of reflecting skilled PT services requirement
gait training to facilitate improved step length and balance for safe community ambulation
a daily note (POC) should include
interventions/progression planned changes specific plans for re-eval Specific aspects to monitor precautions specific parameters (to be modified, followed or changed) a PT or PTA should be able to follow
Patient modifications for paraplegia
MMT -where do they have function and where do they not
sensory
limited mobility and stability
Patient modifications for recent colostomy
avoid incision/bag with gait belt
avoid/limit prone position and (L) side-lying
caution with bed mobility and transfer
focus when you only see the pt for one visit
most crucial tests and measures, focus on intervention and home management
POC should include all additional resources and referrals
