General appearance
Integrated skin exam
Vitals: Pulse, BP, and Resp
Head
Inspect skin Palpate Sinuses Inspect size and shape Test facial sensation and muscles of mastication Test muscles of expression
Eyes
Visual Acuity Inspect eylids, lashes, bulbar and palpebral conjuctivam sclera, cornea, anterior chamber, and iris Test extra ocular movement Assess pupils Red reflex
Ears and Nose
Inspect auricle and external ear
Otoscopy
Assess hearing via finger rubbing
Examine external nose
Mouth
Inspect lips, bucal mucosa, floor of mouth, tongue, palates, tonsils, and posterior pharyngeal wall Inspect teeth and gums Test palatal rise Test tongue symmetry and protrusion Palpate parotid glands Palpate TMJ
Neck
Inspect for symmetry and masses
Palpate neck, include lymph nodes: anterior and posterior cervical, submandibular and supraclavicular
Advanced Chest
Inspect respiratory effort and posterior skin
Assess symmetry of excursion and fremitus
Percuss posterior chest, spine, and CVA
Auscultate posterior, anterior, and egophany or whisper pectoriloquy
Advanced Cardiovascular
Inspect JVP, Precordium for abnormal pulsations, anterior skin
Palpate apical impulse, lifts, thrills, carotids, lower edema, dorsalis pedis and posterior tibialis pulse
Auscultate RUSB, LUSB, LLSB, Apex (with both), carotids
Abdomen
Inspect contour and skin
Auscultate
Percuss 4 quadrants and liver
Palpate 4 quadrants, liver, spleen, LAD, femoral pulses
Neuro
Assess person, place, and time
CN in exam
Motor: upper and lower
-Upper: Shoulder Abd; arm flex and ext; wrist flex and ext; finger abd and flex
-Lower: Hip flex, abd, and add; knee flex and ext; foot dorsi and plantar
Reflexes: bicep, tricep, and brachoradialis; patellar and achilles; plantar
Sensation: symmetry, Rhomberg
Cerebellum: Finger to nose, Heel to shin, and gait
