What is a dispensing error?

A

When an error actually reaches a patient. Patient safety is compromised. Pharmacist could face criminal prosecution or fitness to practice, subject to disciplinary action from NHS and can be sued by patient

What is the professional duty of candour?

A

Must be open and honest with patients. Apologise and offer appropriate remedy or support. Needs to explain short and long term effects of what happened.

  • moderate or severe harm which was avoidable
  • deliberate attempt to cause harm
  • failure to act when necessary to protect safety of patient
  • unsafe working practices
