What are the different NSAIDs?
- Aspirin
- Ibuprofen
- Paracetemol
- Naproxen
- Diclofenac
What are the side effects of NSAIDs?
- GI disturbance
- Increased risk of bleed
- Increased risk of MI
- Increased risk of renal failure in compromised renal blood flow
What are the uses for NSAIDs?
Analgesia
Anti-pyretic
Anti-inflammation
Anti-platelets (aspirin)
What drug is commonly used to treat backspin?
amitriptyline (tricyclic antidepressant), back pain respond poorly to NSAIDs
When are NSAIDs contraindicated?
- History of GI bleed
- Peptic ulcer disease
- Renal dysfunction, HF or hypertension
- Anticoagulants
- Pregnancy (esp ibuprofen)
Give examples of DMARDs?
- Methotrexate
- Leflunomide
- Anti-TNF antibodies = infliximab
What are the actions of DMARDs?
- decrease swollen joints
- reduce pain score
- decrease serum IgG
What is methotrexate used for?
- immunosupression (folic acid antagonist)
- Crohn’s
- cancer therapy
- Rheumatoid arthritis
When is methotrexate contraindicated?
- pregnancy
- causes neural tube defect
What are the side effects of methotrexate?
blood disorders liver cirrhosis nephrotoxicity GIT damage Neural tube defects
What are the side effects of leflunomide?
anaemia
hepatotoxicity
diarrhoea
nausea
When are anti-TNF antibodies used?
1) after 2 other DMARDs tried
2) for Crohns or Rheumatoid arthritis
What are examples of synthetic glucocorticoids?
prednisolone
dexamethasone (most potent used as last resort)
What is the natural glucocorticoid the body makes?
hydrocortisone
What mineralocorticoid is made naturally by the body?
aldosterone
