Pharmacology of Antihypertensive medication Flashcards
what are the goals of antihypertensive treatments?
- reduce systolic pressure to <140mmHg
2. reduce diastolic pressure to <85mmHg (<80mmHg for diabetics)
what drugs are used to treat hypertension?
- Angiotensin Converting Enzyme Inhibitors (ACE inhibitors)
- Angiotensin II receptor antagonists/ blockers (ARBs)
- Alpha-blockers
- Beta-blockers
- Calcium channel blockers (CCBs)
- Diuretics (Thiazide)
what drugs are more effective on elderlies?
Thiazides (diuretics)
what drugs are least effective on elderlies?
betablockers
what drugs are least effective on black people?
ACE inhibitors
what are AB drugs and what do they do?
- AB drugs are ACE inhibitors, ARBs (Angiotensin II receptor antagonists/ blockers) and Betablockers
- they inhibit the renin-angiotensin system
what are CD drugs and what do they do?
- Calcium channel blockers and Diuretics (thiazides)
- reduce peripheral resistance
what drug is started with to treat hypertension in young patients (<55yrs)
ACE inhibitors
what drug is started with to treat hypertension in older patients (>55yrs) or in Afro-Caribbean patients?
Calcium Channel Blockers
during combination therapy, what three drugs are commonly combined?
ACE inhibitors (or ARBs), Calcium channel blockers and Diuretics
why might treatment fail?
- lifestyle (alcohol, exercise, diet)
- Ineffective prescription dose & compliance
- volume overload (renal problems)
- unsuspected secondary cause
