Pharmacology: Drugs And gastric Acid Secretion Flashcards
How do you treat Gastric acid secretion?
- antacid
- H2 antagonist
- proton pump inhibitor
- triple therapy
- mucosal protectants
Conditions treated by Inhibition of Gastric Acid Secretion?
Gastric-oesophageal reflux disease
Functional dyspepsia
Duodenal/stomach ulcers
What are antacids?
Neutralise acid, inactivate peptic enzymes
- magnesium salts: Mg hydroxide, Mg Trisilicate
- aluminium salts : Al hydroxide
What are histamine H2 antagonists?
Ranitidine: competitive inhibitor of H2 receptor in parietal cell
Promotes gastric ulcers healing , reduces histamine-induced cAMP
Adverse effects: diarrhoea, headache, dizziness
What are Proton Pump Inhibitor (PPI)?
Omeprazole :
Irreversible inhibition of H/K ATP-ase
Adverse effect: GI disturbance treated by:
Lansoprazole
What is triple therapy?
Confirm H pylori
Proton pump inhibitor
Clarithromycin
2nd antibiotic: amoxicillin or metronidazole
How do antibiotics work?
1- protein synthesis - clarithromycin
2- cell wall synthesis - amoxicillin
3- DNA - Metronidazole
What does clarithromycin do?
Inhibits growth- bacteriostatic static by inhibiting translocation - prevents transfer of tRNA containing amino acid sequence from A to P site.
What is the mist common used penicillin?
Amoxicillin
What are probiotics used for?
Stool therapy
What are mucosal protect ants agents?
Bismuth
Sucralfate
Misoprostol
What are the differences in Cox inhibition?
COX 1- INHIBITION: unwanted effects including irritation of upper GI tract
COX 2 - INHIBITION: anti inflammatory, analgesic, antipyretic, so make COX 2 selective agents
