The early identification of potential problems and a menu of remedial actions can
increaser agency accountability and offer members a better opportunity to meet the agency’s values and mission statement.

When a member is the focus of three administrative investigations of misconduct within 12 months:
an Intervention Review shall be initiated.
When a member received three counseling forms within a six-month period:
an Intervention Review shall be initiated.
When a member received two reprimands or suspensions within a six-month period:
an Intervention Review shall be initiated.
When a member is the subject of two use of deadly force or shooting incident investigations within 12 months:
an Intervention Review shall be initiated.
When a member is determined to be at fault in three preventable vehicle collisions within 12 months:
an Intervention Review shall be initiated.
When a member uses more than 144 hours of sick leave and / or leave without pay (other than FMLA or Workers’ Compensation) within 12 months and the leave cannot be explained by an obvious reason such as a major illness, pregnancy, or military leave:
an Intervention Review shall be initiated.
When a member is involved in any combination of three of the aforementioned items within a 12 month period:
an Intervention Review shall be initiated.
If intervention is initiated, the bureau commander or designee shall send a follow-up memorandum documenting the member’s progress toward completion of the intervention every – days.
